@Dolle welcome to BYC. Unfortunately, this is a post frm a decade ago, the original poster last seen over 8 years ago. A couple others to0k care of distribution for a while (and two versions of Excel ago) but nocan no longer locate their copies. Which is the long way of saying it seemingly no longer exists.
The formulas themselves are simple, formatting likely takes longer. Looks to be about 2-3 hours in excel to generate this if you were to start from scratch. Maybe if there is sufficient interest, someone will take it upon themselves to devote a few hours to the community. [No, I'm not volunteering, would take even longer to scrub the meta data incluiding personaly identifying information - I've made that mistake once already, I don't tend to repeat it]
