Looks like a sore similar to a bed sore because his hip bone has no meat on it. He needs food. He's extremely emaciated.
He was worse. He's gained about 50-100lbs since my care. Went he went into QT they thought he would die. He put on 200lbs there. He's come a long way. We walk him around on a lead to get fresh spring grass not growing in the pasture. He is in a look no see pen/paddock. I want to get light hoof to install in the stalls we are working on building
 
He still has a long way to go.....
Hopefully he gets there. I know you have to be careful not to feed to much rich foods initially. I bought a very emaciated pony back 30 years ago. Took about 6-12 months to turn her around. Free access to a good grass hay will help.
