Can some one help me identify this disease ? (pictures inside)

I also agree- fowl pox. Just wrote a draft proposal for research in Kauai on the feral chicken population (they are likely to be latent carriers, spreading it to other avian species)- more for practice than for actual funding, though a girl can dream.

It generally does not affect chickens as badly as other avian species... usually the lesions heal and disappear (though it can take awhile - depending on the individual bird). It's more common in young birds and lesions are usually on the eye and beak, as you can see.

There is no reason to vaccinate the flock at this point, or to cull anyone. Exposure leads to resistance. Any one of the birds could be a latent carrier and never show signs of disease (or it could have come from a wild bird- chickens can pick up just about every variant of avian pox, and there are many, many genetic variants). Just keep an eye out for pecking/picking and segregate any birds who are being picked on until the lesions heal. If first aid is needed due to cuts/scrapes, pecks, etc, on lesions just clean and treat like any other wound.
