Chick hatched but died soon after... why? (warning, photo)

Our first chick was perfect , but it died. It was under mama hen. Wet like it was just born. It was about 9:00 pm. Shell was cracked and hen ate the shell. I took other eggs out to incubate. First time for me and this hen. She sat on them and was a perfect mama. Not sure what happened to chick?
