I have come up with a theory. I think that one of my chickens, the biggest, can push open the entrance, and the smallest, an easter egger, can fly over. I came home today to find my biggest chicken had escaped, and then entrance was pushed open. None of the other chickens were out. I still don't know for sure about the easter egger flying, I will watch more. Thanks! 

Personally, I don't want to clip their wings, and I do provide them a covered run for protection. We have lost many birds in our area to hawks.
