Something could be stuck, she could have gape worm, it could be a multitude of things.

Can you look down her throat with a flashlight and see if something is stuck? Take a sniff while you're at it to smell if her crop may be impacted. Have you ever wormed her?

When you look down her throat try to explain everything you see. How long has this been going on?

She could have a respiratory disease, be too hot, or have something in her throat. You may want to post in the Emergencies section for more answers: Emergencies / Diseases / Injuries and Cures
 
If you post on the emergencies thread please do so in detail - some people even post videos of how their bird is behaving.

I'm very sorry she passed. Did you ever get a chance to take a look down there to check for a blockage? How are your other chickens looking?
