Design for chicken safety

We just lost our second chicken to a broken neck. We have lost all 3 of our original chickens now in just over 3 years and they are safe from predators! So I can't help but wonder if we have a bad coop design? Do you know of any reason why neck breaks could keep happening like that?

Also wonders how you are sure their necks are actually broken and not just limp in death?

Is your coop locked? Could a neighbor be killing them?
That's an odd conclusion to draw based on very little information.
 
They can get spooked and fly into a wall and break their neck, it's not too uncommon......
....way, way, way more common than two legged chicken murderers.
