!!Don't know what's wrong with my hen Please help!!

I recently moved from my farm and had to get rid of my flock so I only kept one hen (I saved her from her mother . The hen was pecking at her it looked like she was trying to eat her so I nursed her back to health and she's been healty and spunky for 7 months now) she never really hung out with the other hens in the flock outside she was mostly indoors (her own choice). She's become a real treasure to me.

But recently she's not herself . Although we moved she still laid eggs she was still herself . But sins last week 06/06/2015 She's sleepy all day long and one moring (the same moring all this started 06/06/2015)she jumped out of her nest box and it looked like she slipt and lost her footing falling face first on the ground , at firts look I thought nothing of it untill I saw her waddling around lifting her legs really high trying to walk and only falling over when trying to walk she can stand , hold her own weight and balance herself without using her wings. Her dropings seems normal now at first it was a bit watery (for about and 1-2 hours after her accident) She was on ''bed rest'' for a day and a half and walked like usual after. She eats and drinks normally . As if today 09/06/2015 She still sleeps more than usual she takes alot of brakes sitting down or taking a nap her crop feels normal her vent is clean and has no infection she has no lumps or is hard anywhere on her body . She has lost a small amount of weight and seems sad and she's much more quiet then she usually was and sneezes alot and her eyes seem droopy and she has a scab under her one foot . She seems to be resting her legs alot.

Please help I've never seen anything like this and I have no idea what this is and I'm worried about her . All info and suggestions would be much appreciated.

The weather has recently changed dramatically from warm to cold could this have an effect on her.
 
The weather could have an effect on her, since droppings and everything seem normal I would start with giving her electrolytes and vitamins, polyvisol is a good one or you could give her childrens pedialyte. Is her comb and wattles bright like they normally are? this is where I would start. Hope this helps friend!
 
Thanks so much for your help I gave her electrolytes and vitamins and The weather is a little bit warmer I'm making sure she's outside more by sitting with her other wise she just comes running back inside ... I'm so happy to say she's much better
