What do I do what is wrong

It wasn't ready to hatch.

Make sure there is noting in its way where it can rub up against and break that sac. It will die for sure if it gets broken.

Put it in a tall padded glass and wait. Hopefully it will absorb.

It's not suppose to fall off.
It needs to absorb in to the belly.
