Duramycin-10 dosage for one chicken and possible ileus

Hi,
I have one chicken that I need to give antibiotics to but the pouch I have is 6.4 oz and I don't know how to dose for her. She is 5 lbs.
The package says a 6.4 oz package will make 100 gal containing 100mg of tetracycline hydrochloride/gal.

I just want some water to mix with porridge to make a mash that I can give her to eat. How many times a day and how much water should I mix with the mash.

Some one just told me to mix 4 tsp of the powder into 1 gallon and give her some mash once a day.

Basically her butt feels kind of fluidy and the vet mentioned ileus. She didn't have any bacteria in her poop but thought she might still have a bacterial thing going on so we are giving her the antibiotics and Metacam (for discomfort). Her exam went well, no masses or anything.

Any help or ideas is appreciated.

"A stock solution of 71.4 g dissolved in 1,500 mL (approximately 50 fl. oz. or 3 pt.) of warm water provides about 34 mg of tetracycline hydrochloride activity per mL.

This stock solution metered at 1 oz./gallon will provide drinking water which contains approximately 1000 mg of tetracycline hydrochloride activity per gallon."
