Emaciated hen

4yr old Americana hen. Never laid a single egg and is skin and bones. No other health issues other than never gaining any weight. She has been wormed multiple times with different products. Free of mites.
Extra, extra protein, unlimited feed and fresh fruits and veggies.

I am stumped on what else to do to put weight on this girl. With subzero temps she has to stay inside so I know she is eating well.
