Since you do not have a rooster sometimes a hen will become more dominate. They will act out the mating ritual as a show of dominance. You will see similar damage as in your picture.

Yes The run is completely fenced in and across the top, however the fencing is wide enough for a rat to get through. We lock them up in the coop at dark. Haven’t seen any pecking she is up in the top 2 pecking order

Looks like pin feathers are growing in where the feathers are missing. My guess is molting.
