We had that but it was called Kindred Home Health services but the administration lady came to the hospital & met with us so by the time we got home everything was ready I just had to phone & tell them ready

I guess a good example is when you go #2 and you need Tuck's or ice to make it better ouch

We set hubby up for in home rehab he did weight lifting(dumb bells)upper exercises was with one individual & then another would come out to do his lower quadrant walking,leg lifts,squats,balance(standing on 1 foot)
Upper quadrant neck exercises,arm stretches with weights,hand eye coordination(finger to nose quickly) alternate left right
Then throwing exercise ball back & forth
There's alot involved but it's worth it they would check vital signs & diet each time they came out.
They gave me a journal to keep track of things when they weren't there a book to keep viable information dr appt stuff like that
