Anyway, regarding dressings. When working clinical, I had a lot of treatments to do at night. It was easier to catch people in their rooms so we could get them done. We operated on the premise that if it isn't documented or initialed, it wasn't done so we religiously carried a permanent pen or Sharpie and every dressing we changed or patch we put on a wound, we dated it and put our initials on it. So yep, good nurses who want to get credit for doing their job and cover their backsides while they are at it somehow mark the dressings they do so there are no questions concerning who did the treatment. If a dressing is un-initialed and dated there is no way to tell how long it has been there.
