1) What type of bird , age and weight (does the chicken seem or feel lighter or thinner than the others.)
A pullet, barred rock, 17 weeks old. Yes it did feel thinner than the others.
2) What is the behavior, exactly.
She was letargic, tilted her head, was, reluctant to move, had diarrhea (watery, green poop, then it changed to pure white and dry), decreased appetite and increased thirst, then I suddenly found her dead.
3) How long has the bird been exhibiting symptoms?
Around a week, then it died.
4) Are other birds exhibiting the same symptoms?
Apart from the greenish colored poop, the other birds never showed any signs. The entire flock stopped pooping green once I stopped them from eating moss, and the poop returned to normal with the others, but hers was pure white and dry.
5) Is there any bleeding, injury, broken bones or other sign of trauma.
No.
6) What happened, if anything that you know of, that may have caused the situation.
At first I thought the green poop was due to eating different greens, from vegetables to moss, then I realized something was wrong when the pullet started to act strange. Vet believes that she had a pasteurella infection, which does seem likely, but I also heard it could be some kind of cancer, considering she didn't respond to antibiotics and the other birds seem fine.
7) What has the bird been eating and drinking, if at all.
She refused to eat, but she didn't stop drinking water.
8) How does the poop look? Normal? Bloody? Runny? etc.
At first it was watery and green and then white and almost dust-like.
9) What has been the treatment you have administered so far?
I separated her from the others. The vet treated her with enrofloxacin and ambroxol.
10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet?
She died so we can't treat her, the other chickens are still under treatment, I want to get second opinions on what may have caused the disease. In order to prevent it in the future.

12) Describe the housing/bedding in use.
Due to a coop being under constructions, the chickens sleep inside the house in a 1x1 m box, with chicken wire on top to allow air to flow, the bedding are pine shavings, they spend the day outside in a chicken tractor.
