I once had an art teacher who couldn't remember any of our names. She'd write exactly the same thing for all of our reports, just replacing the names, even if not all of us did the activities she had mentioned. It was quite amusing, actually.
Whenever the students needed help, she'd sigh and act like she'd just been asked to construct a bridge with her bare hands.

Now, THAT was one bad teacher. She retired a couple of years ago, thankfully.

Teachers (or coaches posing as teachers) that are more conserned with their jocks making the grade than helping the regular or unpopular kid who may be struggling with math.. and when they ask for help, don't look at the kid like they have a hole in their head and blow them off.

My english teacher sucked. I was a snooty know-it-all that had it out for me. He actualy pronounced steak like it would rhyme with beak and country like boundry, but with a T( count-tree). (that was back in germany) needless to say that i didnt take english in 8-9th grade.

teachers who are unwilling to work with a child with a known learning disability, for example won't provide written notes for a dyslexic kid

teachers who don't cover the material, but expect students to know it anyway
teachers who are completely inflexible; in time, attitude, approach to students and material
teachers who cannot connect with students or parents
teachers who pick on students due to race, religion or income
