HELP PLEASE! MY HEN IS SICK!!!

I had a hen with a permanently poopy butt and had to give her baths once every week or two. She was that way since she was a tiny chick. The baths really seemed to help her. If I left it too long, she started to look droopy. If you decide to bathe her, be sure she is fully dry before you put her outside. My recommendation would be to blow dry her completely (my hen did not object to this as long as I kept the hair dryer on low) and then put her in a crate in a quite part of your garage and see if she starts doing better.
 
It could be a number of things, perhaps intestinal parasites? That would explain the diarrhea, weight loss, lethargy, and long-running symptoms. You could try a de-womer after doing some serious research on the topic.
 
With that type of weight loss, and with the constant poopy butt, watery poop, I am going with worms. You need to do several things..

Bring her where she will be WARM, at her current weight and condition, she can't keep herself warm.
Get a wormer, safeguard, ivermectin... worm her.
Feed her soft high protein meals. She's serioulsy going to need it.
Poultry drench.
 
Then why is there poop on her but?

You mentioned that she can't get on her perch. If she's sleeping in the bedding or in the nest, she's pooping on herself all night and that may be one reason her butt is dirty.

Good suggestions regarding worming her. You may wish to add some apple cider vinegar [with"mother"] to her water. 1 Tablespoon per gallon. Make sure you use a plastic waterer as the ACV will react with a metal container.

She has been able to get on her perch except just recently. I gave her a bath and got all the poop off. She is in a box and every time she poops I change the towle that is in her box. She already has apple cider vineger in her water. I am planning to give her Ivermecton.
