Hi, my hen was acting off for last couple of days and staying in coop, I just presumed it was broody behavior but today she is VERY sick and I cant take her to a vet until later
1) What type of bird , age and weight.
Silkie, couple of pounds and 2 years old
2) What is the behavior, exactly.
runny poop, dirty vent with wet poop around it, looks like her neck is broken and having trouble holding head up, not sure how long she hasnt eaten for
3) How long has the bird been exhibiting symptoms?
at least one day but could have been yesterday as well, not more than 2 days
4) Are other birds exhibiting the same symptoms?
no
5) Is there any bleeding, injury, broken bones or other sign of trauma.
not that I can see
6) What happened, if anything that you know of, that may have caused the situation.
no
7) What has the bird been eating and drinking, if at all.
not sure but by the look of her there is no way she could eat now
8) How does the poop look? Normal? Bloody? Runny? etc.

9) What has been the treatment you have administered so far?
None but will give some poultry drench ASAP
10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet?
