Help !

My sapphire gem hen twilight has this strange thing hanging out her butt and it’s covered in blood she is acting very sleepy won’t eat or drink and is just sitting in a cornner

While you're waiting for an answer from the Educators, mix her up some sugar water and hold it up to her beak to drink. 1 c water, 1 tsp sugar, a pinch of salt or baking soda. Put her in a dog crate in a dark room, she may be in shock and this will help. Offer her the sugar water about every 10 minutes. She appears to have a prolapsed vent. Coat the tissues hanging out of her with Vaseline so they don't dry out.

The picture is out of focus, but it looks like her vent has been pecked pretty bad. There may be a prolapsed cloaca, or blood dripping, but be sure to note if any intestines are coming out. If that is the case she might need to be put down. Spray her vent area off in a dish pan, sink, or tub, so that you can see it well.

If she had a slight prolapse laying an egg or straining to poop, others may have seen the red and attacked her. Sometimes a bully will vent peck other chickens. If she still has a prolapse it will need to be treated, cleaning, and keeping it covered in vaseline or antibiotic ointment, so that it does not dry out. Look for any bulge that might be a stuck egg. Post another picture if you can. As Big Blue Hen said, offer her some sugar water or electrolytes.
