Hen, fast decline, lethargic

I have a 2 year old New Hampshire hen, who this morning laid an egg, and did her egg song for about 20 minutes (10am). I just went out to check on them and she was laying down. I coaxed her up with a Japanese beetle. She then wasn't interested in a second, or any food. I brought her water. No interest. She just stood there, eyes half closed, letting the others peck her. I could hear her breathing from 1.5 feet away. I tried to catch her, but couldn't. I gave her a few minutes to chill out so I could try again. In that time she started pecking at the ground, so I gave her a few mealworms to see how she was feeling. She ate them but after them the kept swallowing even though she had already swallowed the mealworms. Then she just kept standing there, with her eyes partially closed, while the others were going to bed.
She seems unsteady on her feet. Every once in a while she acts like she's on a perch that has been bumped.

What are you feeding including treats?
Where are you located in the world?

Photos of your birds and their poop?

I would inspect her really well - see if there is another egg inside the vent, look for a discharge, check her crop and let us know what that's like, Look inside her beak for any mucous/canker/yellow pasty material and look her over for lice/mites too.

You had another thread earlier in the week - you had hens gaping?
How much ventilation do you have in your coop - you also mention that they didn't have food (?) for a while and possibly they ate too fast?
Do your birds have access to fresh food and water during waking hours? Have they gone without water as well?
Do your birds have access to grit?

Safeguard is dosed by weight - if you were wanting to worm them, then I would dose them at .23ml per pound of weight given orally for 5 days in a row. This will take care of most worms that poultry have.

She is 2 years old.
They get Dumor 15% Grower Finisher, and mealworms, they get scratch also but I haven't given any in a while.
I am in upstate New York.
I have 8 vents and a window in the coop, which are all open, but they currently sleep in the run.
I don't think they've ever gone without water for more than 15 minutes at a time. I'm not so good at keeping the feeder they prefer full, they have other feeders that they barely use, and they are free ranged for about 12 hours each day.
They have access to grit but I don't think they are aware of it's existence, I'll try moving it to a more obvious spot.
With the Safegaurd I did 3ml/gallon for five days then fresh water for a week then another 5 days. From what I read that seemed like another viable option. However they're still gaping a bit.

I went out before breakfast this morning, and she seems to be acting like her normal self, ate some mealworms, and refused my attempts to touch her.
While I was eating breakfast she did the egg song for like 15 minutes.
I then went out about half an hour later and she was lying down under the coop, no egg to be found. As I'm writing this she's moving around the run, and looking to jump up on a perch/ into the coop.
Here is a picture of the chicken in question, and the area underneath where they sleep.
As I was getting the pictures I noticed she was shaking her head every few minutes.

Thank you for the suggestions. I've noticed that she appears fine for most of the day, but a times, gets sleepy and not her normal self. I know that sounds like she's being a normal chicken but during these period she's tired, which I don't think she's ever expressed before.
Would any of the possible problems that you have mentioned present like this? She really doesn't like being touched, so if there's something to look for I want to do it all at once, and if there's something that gets ruled out by this, I don't want to put her through me looking for it.
She has a definite appetite, and most of the time seems to not have any problems vocalizing and is just as loud as ever.
One thing that I forgot to mention originally is that she has been crowing. And still laying eggs. I don't know if that points at anything or not.

Go out early while she's still roosting and grab her off the roost. Check her then - feel her crop, get a good look at her droppings overnight, look her over for lice/mites, look inside her beak.

Checking out the overall health and function of the bird is needed, then go from there. The symptoms you describe can be a number of things - you have to start somewhere.
