Hen is lethargic and not acting herself. Please help

I am raising chickens for the first time and might be facing my first issue.
One of the 5 chickens is not being herself, is just sitting and not active like it used to be. Any help is appreciated. Its stool is watery, <Graphic pic alert>.

I gave her some crushed garlic/turmeric/oregano oil 2 days in a row, had to feed it in and some electrolytes, she seems to be doing ok and moving and eating ok for now, also waiting to see if she lays an egg to make sure she is totally ok.

Do you have a food scale that you can weigh her on?

Yes we do have a food scale. I will weigh her when I get back home and respond back.
