Hens face is reddening.

She is reaching sexual maturity and will start laying eggs soon. It is a surge of hormones that triggers the comb to grow and swell and the face to become more red and sometimes the legs to turn red too almost like they have a rash. It is natures way to indicate to any roosters that she is becoming fertile and ready to breed, although of course, she will lay eggs regardless of a rooster being present. You should see eggs from her in the next few weeks.
 
She's starting to mature. She will pink up/redden more as she gets older.
At 4 months, she is probably not quite at point of lay yet.
