Yep seen pics of that once before.

Just a glitch in the egg machine, can be caused by stress or a scare but not necessarily.
One egg is completed and travels back up the chute and another, this time a double yolker, is formed around it.

The egg came from a golden comet pellet.

I think Golden Comet is a sex link cross bred for gender separation at hatch and high egg production....not surprising for her high capacity egg lay machine.
