That looks like a classic bumblefoot scab. You would still do the Epsom salts soaks, and then work on getting the scab loose and peeled off over a few days. The surgery is quicker, but there are many different approaches. Some use a sugardine dressing on the foot for 2 days, changing it every 2 days. Sugardine is simply Betadine mixed into a paste with sugar in a container. Apply it to the scab, cover with a folded 2x2 dressing, then wrap with vet wrap or cohesive bandage strips. I usually only do the surgery if the chicken is limping, and the bumble looked abscessed.
