I would suggest doing little to nothing at this point. Monitor, make sure nobody's picking at it making it worse and let it heal. If it worsens, that would be the time to begin treatment. If you absolutely can't keep yourself from doing anything at this point you could cleanse it and apply a little triple antibiotic cream, but the more attention you draw to that area the more you increase the likelihood that others in the flock notice that area which then could potentially lead to picking

Do you have a rooster? The scab and bare spot looks to be healing, but the location and look of the scab along with the bare spot, worn and broken feathers reminds me of overmating or a hen being mounted and plucked.

If you have a rooster that is overmating her, you can move him for a while until she heals. Her worn and broken feathers will grow back once she molts. Alternately, you can put a hen saddle on her to protect her back, but the skin under the saddle needs to be inspected a few times a week to ensure the saddle is fitting well and there's not issues.
