Introducing single hen

We've been doing that. So recap.

Week 1 - in dog pen outside while others free ranged around her.
week 2 - free range together until I lost one to a hawk, cooping them up together at night. When they free ranged, the pack would be in a completely different area, and new girl was on her own.
week 3 - dog cage inside run, cooping together at night
week 4 - attempting to let them inside the run together, but she just hides in a nesting box. I attempted to put some more hiding spots in the run, but she refuses to come out, and the others attack her as soon as she sticks her head out. The last two days, I've been locking up the head hen in the dog cage, so she can only get picked on by one, but she still just hides in the nesting box unless I lock them out of the coop.

How big is your coop and run, in feet by feet?
Pics would help immensely.

You may need to start over with her in a crate for longer than a week.

Coop is 4x4, run is 12x12.

This is a picture when it was first built, there is a lower overhang on the side where we put the food. There's a bit of things in there for them to sit on now and they go on top of the coop, too.
