Is something wrong with with her?

Hens will do that. If your chicken with the missing feathers is at the bottom of the pecking order that could be the cause.
She’s on the lower side of the pecking order, but not the lowest. I know that just because of where she sits on the perch and the |-/ Cliques |-/ she is in. I’ve also seen her being bullied a lot. The lowest 2 on the pecking order is my hen that recovered from Sour Crop today and little Nugget.
 
I agree with @dawg53 the white on her skin I would apply an anti-fungal cream.
You have a hen that acts like a rooster, so some of the feather loss on her head could be due to that.
The loss around her front...watch to see if she's pulling at that and check her crop to make sure it's empty in the morning before she eats/drinks. She may be plucking at her crop or she's getting picked at during roosting.
 
I have a couple of ex battery hens that are technically the same breed. A couple of them lost there feathers on there heads whilst not in molt. They did grow back when they did finally molt. I'm not sure about the head shaking. Check for mites and check she has clean ears.
 
I actually do have a hen that ACTS like a rooster. She has spurs and when she bullies the hens, they squat and she gets on top of them and they no joke look like they’re mating.
that would be a likely cause for the missing feathers if you have a hen acting like a rooster, plus her scratching, could be bugs or dry skin, I know especially in the heat my hens seem to have extra dry skin and look like they have dandruff from scratching the itchy dry skin
