I was raised to say Yes/No Ma’am and Yes/No Sir.
Also to use Mr. or Mrs/Ms. before names.
And I still use this when addressing my parents and anyone else of their age or older.
My children were taught the same and they still use this even as adults.
There are children and teens in my town who still use ma’am and sir.
IMO it is a form of respect, towards our elder and wiser group of people, which has been lost.
To my knowledge, no one in our town has yet to be offended by it.
 
Here in Michigan, "yes please" and "no thank you" were what were drilled into my head. Ma'am, Sir... not so much.

Very interesting. I would not have known that I was being rude if I didn't say Ma'am/Sir down south.
It's such a big deal in the South! When I go to visit family there EVERYONE is ma'am or sir....or at least in the small town I was in.
