I’ve only had it happen when I used the insert multi button. Maybe that’s it?

It's a picture I took of my dog. I uploaded that one from my iPad - though I have had trouble uploading from my computer too which I use file explorer. On my iPad I just click "upload photo or video" click the one I want from my camera roll, then click "full image" and then "post reply."

I've never seen that. So I don't think that would be the issue.
