I would take a form if possible from the ESA site and explain that you really need the support from this hen. If it were me I would want the hen to help me get through a senior year. Senior years can be stressful enough living in a non toxic environment.
Yes I was thinking about using the trauma of loss and depression from the other chicken we had, we used to have two but that one passed away the first day of school back then my 10th grade year :( I could use that since that is true. I’ll say how this chicken helps me cope that pain and allows me to forget by giving me new and joyful memories.

By all means only the truth. Any lie would come back to hurt you for a long time.

Update: the website only has an option for cats and dogs, I tried leaving it blank and put a picture of my chicken next to me while I did homework outside with her, but it didn’t let me pass unless I filled in the option, and the only option was a cat or a dog.

I'd go to the doc without a form and tell him you don't need a cat or dog just the chicken that helps with the loss of the other chicken and hope you get to move soon. Give your sister some credit that she can give yours medicine for a few weeks if necessary.
 
So for this, can I just go to them in person or do I need to show them something because Triciayoung she was kind enough to give me a link about registering our pet to an ESA. By that, do we have to register our chicken first or could we just go to our doctor and ask?
Online registration is usually a scam. Go in person.

You have to register for it to be done.
