Lethargic hen at 13 weeks old

I am new to raising hens and have 4 that we bought at 10 weeks old. One is lethargic so I started feeding it a mixture of crumblings, yogurt, applesauce, milk and a little honey. She seems a little better but, her eyes are still shut at times. What else should I give her and how much. She seems clean without worms or an infection in her bottom?

Could she have coccidiosis? It makes them lethargic and puffed up. Easy to treat. Corrid can be found at tsc and is mixed right into their water!
