Yea, it sounds like you are doing everything right. Sometimes chicken anatomy just fails and things like this happen.

I'd probably just keep an eye on the rest of the flock looking for anyone struggling to lay eggs, walking funny or avoiding the group. Especially watching hens related to this one (I honestly suspect hereditary for this).
