.... It does not plug in and it came with batteries in it. I just need special directions because I didn't realize I had to remove a small circle thing from on top of the batteries.

Not really. If the air sac is moving that is something I've never heard of, but it sounds like the outer membrane has had time to develop "issues".
My vote, sadly, is that it's a dud.
