Sounds like he has a prolapsed vent. Don't see it very often in a rooster but you need to keep him away from the hens as sometimes this is caused by mating too often. If he is a breeding rooster he needs to be culled. However, if he's a pet you will have to find a veterinarian who has experience in doing surgery on chickens. He will need to have his testes removed (castrated) and his vent reconstructed to prevent it from prolapsing. The removal of testes stops the hormone production which will can cause a rooster to prolapse his vent and penis. Good luck!
