That got really good info. My one hen with foamy eyes on has that one symptom for Mycoplasma gallisepticum.You could cull a sick bird and send it to your state vet to get it tested. What state are you located? MG is a disease that cannbe common in flocks. It can be passed through hatching eggs. Treating a chicken with MG with antibiotics will not keep the chicken from still being a carrier for life. There are about 5 or so respiratory diseases, and some have similar symptoms.
