NPIP paperwork confusion

Okay... so I've gone through the NPIP testing and all is well. I got the card in the mail today. WHOOHOO!! The problem is I thought that there was paperwork too. Aren't I supposed to have paperwork to include when I mail chickens? Or am I just supposed to put the NPIP number on the box?

How do those of you that routinely ship chickens and chicks do to keep it all legal?

Help NPIP gurus!!
 
Not sure....when I requested it from Meyer hatchery to move across country, it was basically an invoice showing the breeds purchased and quantity with the NPIP number.

NPIP is a Federal program administered by the States & they all do it differently. In NY we receive a dated certificate showing participation in the program. I send a copy of that certificate along with any birds I sell if the buyer requests it.

These are the requirements for the State of Louisiana.

if you are selling chicks, eggs or poults out of state, you need to have a USDA form 9-3 filled out which specifies the level of NPIP participation you are in. that form is sent to the state that is receiving your product, so that they can know what and where things are moving into the state. We usually have the vmo of tech that did your test fill a few of those out for you and show you how to do it properly. For trading within the State, a copy of your 9-2 test is adequate.
 
It's called a VS 9-3 form. Call your State Dept of Ag & Markets and ask to get some. I order 2 books of 50 forms each. The person that sends them had to individually fill out every single form (100) with name, address, and then sign it. How stupid is that! Poor guy must have been sitting there for over an hour filling out these stupid forms. I do is fill it out, and stick it in a packing slip on the bottom of the box.
