Sounds to me like she was doing well at first before she really bonded to you. She wants to be near you and its more convienent to come back in to be with you, and also do her potty near you in the house, AND its warmer in the house and she has no coat really.
1) So first thing is to put her on a potty schedule, out every two hours. -- You must go with her to make sure she goes, after she does go give a food reward and lots of praise.
2) When you aren't supervising her, make sure she is crated (get a crate if you don't have one, or, keep her on leash with you)
3) No unsupervised time in the house for several weeks.
This is the only way you are going to get her 100% reliable. You have to have control over her schedule to do this and also the supervision can't be lax at all.
Good luck!
