Over 48 hours since internal pip and no external pip - gone quiet and no movement

Ok I assisted a bit more - realised that he’s malpositioned - foot above head. Membrane was leathery, but also bled a little. So I’ve stopped again, moistened the membrane and put him back in the incubator. Not sure how long to leave it before I help again, don’t think he’s going to do it on his own - but also don’t want to help to soon will he’s still absorbing blood/yolk. When I talk to him he struggles a little like he’s keen to come out.

You gotta remove the shell tonight that's to long to not improve. Use the tweezer slide it between the shell and the membrane. Remove most of the shell, then check for any blood vessel on the membrane. If there's not a lot of veins, peel the membrane back away from the chick. But do it quickly without drying out the chick.
 
Antibiotic ointment without pain relief, like betadine is good for moistening membranes and makes them easy to see through.

Foot over head is an assist malposition - he won't be able to get out on his own.

For the assist, steam up a bathroom or get a nice warm room to operate in, have some styptic powder (there are lots of things that can be used for this - google it and you'll have at least one in your pantry or vanity) and q-tips on hand to stop bleeding, and some tweezers for prying away membranes.

Most of assisted hatching is being ready to react quickly if you encounter bleeding and knowing when to stop. You may not end up with any bleeding, but having a bit of powder in a ramekin to the side with a q-tip handy can be a great time saver if it does happen. Just dab to generously cover the bleeding area in the powder. A little blood may be left in the membrane and show up if it is torn (though peel where you can), but it's not a worry unless it is actively bleeding.

The assist thread already mentioned is great! You should really give it a read any time you're stuck waiting on hatching day. Helps pass the time and keep from jumping in too early. :)

Try to get the chick to a point where it can finish kicking out on its own. With foot over head, it may be a bit wobbly on its feet for a few days, which is normal as long as its making a little progress each day.
