Please help! Emaciated chicken!

I have a hen who has been severely emaciated for about a week now. She recently got over a pretty bad intestinal bacterial infection after I took her to the vet and got antibiotics. I thought she was ok for about a month then noticed she has been declining. She has liquid dripping from vent. It is clear/ dark brown. Her poop is white. She has been really picky and only wants to eat mealworms. I’ve tried everything to help her.

How old is she? Does she lay eggs? How does her crop feel early in the morning before she eats or drinks? What antibiotic has she been on? Have you given any form of probiotics to her, or wormed her recently?
 
She’s about 5. Hasn’t been laying. She mostly stops laying in the winter time. Her crop feels mostly empty with some grit. The antibiotic was LinxMed- SP. I have not dewormed or given probiotics. She does not have mites. I tried to get her to eat plain Greek yogurt but she’s not interested.
