Please help, not sure what the problem is...

The more dark and private you can make it the happier she will be. Don't be surprised if they try to scratch all the nesting material out like they did when they first started laying, so make sure it has a nice big lip on the front to keep most of it in. They like to investigate it thoroughly and make sure it is safe before they leave eggs in it.
 
I filled out the sheet and we are pretty much clear on everything, but on the part where it says, Services Requested, we checked Full Necropsy, but there are a bunch of other options and we aren’t sure if we should check any of them because we don’t know what they mean.

I'm not sure...you may need to request PCR since usually that is used to test for Marek's.
Hopefully @Eggcessive will be on soon, if not maybe Kathy @casportpony can tell us if you need to check any other boxes.
 
I think you only tick those other boxes if you have not ticked the full necropsy box. If you have included your contact details they will be in touch if they need to clarify anything.
Is there a part of the sheet where you submit information regarding the symptoms exhibited prior to death? Hopefully so. They will need to do PCR testing for Marek's

Yes I filled out a part about hersymto s, treatments, etc, and said it could possibly be Mareks

I think the part where is says Full Necropsy or Specific Testing. The or being underlined probably indicates that you don't need to specify but you could perhaps include in the virology box "Marek's suspected" in small letters
