My horse survived getting impaled on a 2x4 and colicing due to an allergic reaction immediately following the injury. Another horse got loose from the neighbors, ran in our barn and bit him in the face until he ran back words threw a wall to get away from her. It was a lot of dr-ing at home and some vet bills but he is still here. She should be on either oral or im antibiotics and perhaps an anti-inflammatory pain reliever as well.
 
I have a 33yr old mare who got tangled in wire several years ago. The wound was horrific. Like yours, it was down to the bone. The front of Gail's hock had a hole in it that I could stick my fist in and not touch flesh. Due to it's location and the time between injury and treatment, stitching wasn't possible. My vet actually said that it would be too much work to get her through it and suggested that I put her down. I told him to give me the instructions and I would decide what was too much work.
I should warn you, it took about a year and half way through it I wasn't sure that the vet wasn't right. It WAS a lot of work, some big disappointments and frustrations and occasionally some very late nights. But she DID come out of it ok.
