Preventive and Medicine Page? I need some help from all you experts!

I have been thinking of making like a page similar to the treat chart, but for like diseases. More for myself, but other people could use it too! Is this already done or is one started? I know there are alot of websites for info but it can take to much time if its an emergency. So, does anyone want to contribute to the chart? If you do try to include...

1. Disease name
2. Symptoms
3. Cures
4. Preventive Care
5. Picture (If you ever had a chicken with the disease)

The Merck Veterinary Manual has everything you need to know. I use it often. Just google it and once your in, click 'poultry' in the table of contents on the left and you're good to go.
