Prolapse in 13 week old chicken when pooping

Hi our 13 week old chicken (pullet or cockerel) has a prolapse which goes in and out we have noticed when pooping. The said chicken always has a messy bum. Any advice?

You can put a little Preparation H on it to give some comfort and hopefully help shrink the inflamed tissue. Do try to make sure it does not dry out.
