I am sorry to hear that you lost her. You really did what was best with keeping it moist and trying to stop laying and minimizing her discomfort. Prolapse cannot always be repaired so it is good that she did not have prolonged suffering.

Very sorry for your loss. Prolapses are sometimes very difficult to treat. Most people used to put chickens down immediately when they became prolapsed. Some cases are milder, but this one was not. Have you ever done a necropsy on a chicken? It sometimes can help to understand what was going on inside. She could have have other problems. If you do one, it can be helpful to post pictures here of your findings. Also a professional necropsy by the state vet is a great way to get findings.
