1) What type of bird , age and weight (does the chicken seem or feel lighter or thinner than the others.)
A paduan, age not known, doesnt feel lighter or thinner than her/his siblings.

2) What is the behavior, exactly.
Head is constantly tilted to the left. Sonetimes completely flipped
. Spams/twitching occurs randomly which last 5-15secs
3) How long has the bird been exhibiting symptoms?
4 days

4) Are other birds exhibiting the same symptoms?
No

5) Is there any bleeding, injury, broken bones or other sign of trauma.
Not visible

6) What happened, if anything that you know of, that may have caused the situation.
Went to check the henhouse in the morning and noticed that she was in an unatural position.

7) What has the bird been eating and drinking, if at all.
Eating and drinking in a natural way. We are giving her aspirin/vitamin supplements, boiled eggs for selenium with her regular feed.

8) How does the poop look? Normal? Bloody? Runny? etc.
Looks normal

9) What has been the treatment you have administered so far?
Multivitamin suplements, boiled egg. Electrolyte water

10 ) What is your intent as far as treatment? For example, do you want to treat completely yourself, or do you need help in stabilizing the bird til you can get to a vet?
Hopefully treat her at home as no expert vets in the area.

12) Describe the housing/bedding in use
Shavings

It seems like wry neck but we are not sure and the intensive spasm throw us off.

I would agrre that she seems to have wry neck, a neurological symptom that can be caused by head injury, vitamin deficiency, is sometimes seen in certain viruses such as Mareks. Read my post in this thread for vitamins and where to find them for wry neck:
