question about vent prolapse

We just found one of our pullets dead outside. She hadn't displayed any sign of anything. Looking her over the only thing I can find is what appears to be vent prolapse. Her vent is distended and bloody. To me it doesn't look horrible and I don't really feel an egg. Would this have killed her? Any insight might help.
 
Was she cannibalized by her cohorts? Was there evidence of mass blood loss?
 
They free range and she was outside on the ground. No evidence of blood loss except blood just around vent with something bloody protruding, not egg but flesh. I had seen her straining yesterday but after she laid an egg that was not bloody I didn't worry about her. She hadn't acted different in any way today and she was not near any other chickens and no sign of cannibalization.
