Rescued duck SEVERE niacin deficiency

Oh wow, that IS severe. :( You're covering all the bases; nutritional deficiency, birth defect, injury...
How old is he? When they're less than 1 week their bones can be manipulated by manually holding them in position with tape/splint etc, but he looks to be older than that so they might be set that way now.
Don't have any additional advice, you seem to be doing what you can for him with the exercise and manual stretching. Hope it works over time! :fl

I could be wrong, but I feel like I heard that if a niacin deficiency isn't corrected soon enough the problems become permanent.

I'd definitely get him into some sort of appliance that keeps him in the proper position at all times. The longer he stays like that the higher the chance he's gonna be stuck that way forever.

Was the duckling ever able to stand?
Is he using the legs in the water?
If his legs are entirely limp and he never stood, he might be paralyzed from birth and then there is not much hope for a normal life, unfortunately.
If he is moving the legs in the water, then you should continue with swimming exercises and high doses of Niacin, he may recover.
I have never seen a duckling sitting like that.
