Rooster with an itchy head

My little rooster was treated for a respiratory infection and all but his itchy head has gone away. He went to the vet and was given antibiotics which helped greatly but he continues to scratch his head which is now bald and raw. He has an appointment to go back to vet in 2 days but wanted to know if anyone can suggest something I can do for his itching. He has no bugs and I have tried hydrocortisone cream.
 
Do you know which respiratory disease the vet was treating? Do you have pictures of his head where it is itching? It is not common for a respiratory disease to cause itching and feather loss on the head. But the eyes do itch and can have drainage. Many ointments and creams that contain menthol can relieve itching, but don’t get it in the eyes.

She said he had a sinus infection and crackles in his lungs. She said the pressure in his head could be causing him to shake and scratch his head. She gave him a dose of anti inflammatory and put him on antibiotics plus aspirin for 7 days. Face swelling went down and no nasal drainage now, just the itching. The first pic is the side that seems to be bothering him the most. He has no feathers over his ear and has lost more on his neck.
