It is a rooster problem you probably have too many roosters or this hen is just the favorite or she could been attacked by a rooster or a predator of some kind

Thank you. We do have two roosters. I need to only have one. I just don’t know if I keep the aggressive one or the calmer one.

I would the aggressive one. I never keep a mean roo.

You want a rooster that respects you and the hens for sure but a rooster will go to mate a hen and the other rooster will see it and they will go back and fourth

and for your hens head, there is this stuff called bluekote i think it will help
