Several poopy butts

I have several chickens with poopy butts and one that is lethargic. I treated them with safeguard a month or two ago after two hens died suddenly and one poop had a ton of worms. I haven’t seen any other worms since, and only that once. Laying is down. Ideas? I don’t want to lose more hens.

I would clean each one in a bucket of warm soapy water (change it after each chicken) to remove the droppings. Trim any extra fluff there that might be collecting droppings. At the same time look for any lice eggs or white to grey clumps under the vent. This time of year during warm weather messy vents can be a result of drinking mire water, but flies are out and can cause flystrike and maggot infestation. When you used the SafeGuard, did you do a followup worming 10-14 days later?
 
I will wash them off tonight. I didn’t dose them a second time. Should I worm them again and do it twice this time?
